Change of Preceptor
If you need to change your preceptor, your new RD preceptor must submit the following to UND Online & Distance Education:
- New Preceptor Application (PDF)
- Copy of the American Dietetic Association CDR Registration Card
Change of Facility
If you have a job change and are employed in the dietary department of a new facility, you may transfer the course to your new facility IF:
- You paid for the course yourself, AND
- You have approval from the administrator and the dietitian at your new facility.
Upon approval from your new administrator, you must submit the new facility's contact information to UND Online & Distance Education.
